PPMD strives to ensure that every patient with Duchenne and Becker muscular dystrophy receives the best care possible. An important part of PPMD’s certification and continued maintenance of Duchenne Care Centers around the country is patient and family input. That is why we have updated our Clinical Experiences Survey and want your help by providing your feedback! 

Please complete the survey below to provide feedback on the care experience at your Certified Duchenne Care Center. If you are seen at more than one center, please complete a survey for each center. If you’ve completed the survey before, it might be time to do it again! We appreciate continued feedback about centers and ask that you fill out a survey for your center at least once per year.

If you are seen at a center that is not currently certified, we still want your feedback! As our network grows, we will continue to evaluate many more centers with the goal of ensuring access to quality care for every individual with Duchenne no matter where you live.
